A Note on the Science

The physiologic mechanisms described in this book—the autonomic regulatory hierarchy, the circadian clock and its peripheral clocks, mitochondrial signaling, the gut-immune-microbiome relationship, and the role of mechanical loading in structural repair—are grounded in peer-reviewed research across the fields of neuroscience, chronobiology, immunology, cellular metabolism, biomechanics, and exercise physiology. The Ettinger Foundation Protocol (EFP) framework integrates these established biologic principles into a clinical sequencing model that is, to my knowledge, original to this practice.

This book was written to make complex physiology understandable and clinically useful rather than to function as an academic textbook. A selected reference section follows for readers who wish to explore the scientific literature supporting the concepts presented throughout the book. The EFP Practitioner Manual expands substantially on these concepts, providing more detailed scientific discussion, additional mechanistic explanations, and an extensive reference library for clinicians and researchers who wish to examine the primary literature in greater depth.

Science continues to evolve. The principles presented in this book represent my best understanding of the available evidence and my clinical experience at the time of publication. As new research emerges, the EFP framework will continue to evolve with it.

To support that ongoing evolution, I have created HealingInOrder.com as the companion resource to this book. There, new research, clinical insights, and refinements to the EFP framework will be organized by phase of the regulatory hierarchy, allowing readers to follow developments in the science without having to search the medical literature themselves. My hope is that Healing in Order remains a living resource—one that continues to grow as our understanding of human physiology grows, while preserving the biologic principles on which it was built.

The patient stories in this book are composite clinical narratives, as noted in the disclosure at the beginning of Chapter 1. They were created to illustrate real physiologic patterns observed across nearly four decades of clinical practice while protecting patient privacy. The physiologic patterns are real. The individuals they portray are not.
